<p style="margin: 9pt 0cm; line-height: 15pt; vertical-align: baseline; font-style: inherit; text-align: justify;"><span style="font-size: small;"><span style="font-family: Arial;">Thí sinh lưu ý, trong túi đựng hồ sơ: Mặt trước ghi các thông tin cần thiết mà thí sinh phải điền đầy đủ, mặt sau là một số lưu ý về làm hồ sơ ĐKDT (đây chính là một phiếu ĐKDT và sẽ được gửi đến trường ĐH, CĐ nơi thí sinh ĐKDT). Phiếu số 1: Mặt trước có nội dung giống mặt trước của túi đựng hồ sơ, mặt sau gồm các thông tin về tên và ký hiệu các trường ĐH, CĐ. Phiếu số 2: Mặt trước của phiếu về cơ bản giống mặt trước của túi đựng hồ sơ, mặt sau là hướng dẫn ghi phiếu ĐKDT.</span></span><span style="font-family: inherit;"><o:p></o:p></span></p>
<p style="margin: 9pt 0cm; line-height: 15pt; vertical-align: baseline; font-style: inherit; text-align: justify;"><span style="font-size: small;"><span style="font-family: Arial;">3 ảnh chân dung cỡ 4x6cm được chụp trong thời gian 6 tháng tính đến ngày nộp hồ sơ ĐKDT; 3 phong bì đã dán sẵn tem ghi rõ họ, tên và địa chỉ, số điện thoại của thí sinh. Bản sao hợp lệ giấy chứng nhận ưu tiên (nếu có).</span></span><span style="font-family: inherit;"><o:p></o:p></span></p>
<p style="margin: 9pt 0cm; line-height: 15pt; vertical-align: baseline; font-style: inherit; text-align: justify;"><span style="font-size: small;"><span style="font-family: Arial;">Riêng thí sinh có nguyện vọng học tại các trường ĐH, CĐ không tổ chức thi tuyển sinh hoặc hệ CĐ của trường ĐH, trường CĐ thuộc các đại học cần nộp thêm bản photocopy mặt trước của tờ phiếu ĐKDT số 1.</span></span><span style="font-family: inherit;"><o:p></o:p></span></p>
<p style="margin: 9pt 0cm; line-height: 15pt; vertical-align: baseline; font-style: inherit; text-align: justify;"><span style="font-size: small;"><span style="font-family: Arial;">Thời hạn thu nhận hồ sơ ĐKDT, lệ phí ĐKDT quy định thống nhất trên phạm vi toàn quốc từ ngày 11/3 đến hết ngày 11/4. Các trường ĐH, CĐ thu hồ sơ và lệ phí ĐKDT mã 99 từ 12/4 đến hết ngày 19/4.</span></span><span style="font-family: inherit;"><o:p></o:p></span></p>
